Patients whose personal information was potentially exposed in the June 2023 Akeela data incident are eligible for two years of identity protection and pro rata cash payments. Claims for these benefits, including credit monitoring and a share of the $50,000 settlement fund, must be submitted by the May 25, 2026.

You may be eligible to file a claim if you meet the following criteria: • Personal Information was compromised in the Akeela, Inc. data security incident discovered on or about June 22, 2023 • Impacted data may include names, Social Security numbers, dates of birth, and medical diagnosis or treatment information • Not an officer or director of Akeela, the presiding judge (including their staff or family), or anyone criminally liable for the incident
